Tubal Factor

  • Fallopian tubes plays an important role in the process of conceiving.
  • The egg & sperm meet in the outer half of the fallopian tube called the ampulla.
  • After fertilization embryo continues down the tube towards the uterus and right implantation time is about 4 to 7 days after ovulation.
  • The tube is more complex than a simple pipe.
  • The lining of the tube is folded and lined with microscopic hair-like projection called cilia.
  • Cilia push the egg & embryo along the tube.
  • The tube lining also produces a fluid that nourishes the egg & embryo during their journey in the tube.
  • In this way, the tube has its own local endocrine system.

 

Tubal disorders

  • Tubal abnormalities are about 2 % and nearly 50% of female infertility.
  • Tubal damage usually occurs with pelvic infection mostly by T.B, chlamydia, gonorrhea.
  • Post operative pelvic infection & infection after childbirth & miscarriage & MTP kit is also responsible for tubal disease.
  • Severe endometriosis and P.I.D cause scar tissue & adhesions without any symptoms which can alter the functioning of fallopian tubes.

 

Diagnosis

  • HSG ( hysterosalpingogram ) is used for checking tubal patency, usually, it is carried out on 6th or 7th day of the period at x-ray clinic.
  • Sometimes, it may give erroneous result due to the spasm of cornu of the uterus or very thin hydrosalpinx which tears easily at the fimbrial end and that gives false tubal patency report.
  • HSG provides little information on a structure outside the tubes which might be cause for impaired tubal function such as peritubal adhesions.
  • HSG provides only information about the structure, it does not provide any information about the function of tubes. A tube that is patent does not necessarily mean that it functionally works with fertility concern.

 

Materials of blockage

  • Adhesions of healing of past infection
  • Mucus plug and fibrinous deposits
  • Fluid in case of hydrosalpinx
  • Clotted blood in case of endometriosis

 

Hydrosalpinx & salpingitis

  • Hydrosalpinx & salpingitis is due to infection in tubes, it may be acute or chronic.
  • It is the main cause of peritubal adhesions & fibrosis inside the tubes.
  • Hydrosalpinx always occurs when there is adhesion or blockage is associated with some fluids.
  • Normal tubes secrete some physiological fluids which can be absorbed by tube lining.
  • Due to infection a large amount of purulent fluid can be discharged which is unable to be absorbed or drained out and it damages the function of tubes.
